There are two ways in which you can submit your booklist and receive your items:

  • Mailing us your application by following these steps:
    1. Open, print and fill this order form.
    2. Log-in here using the information provided on your booklist instructions, bring up a copy of your booklist, fill it in and print it.
    3. Post these two documents to the Academic and General Prepack Department (the address can be found on the Order Form)
